Facilities Use Agreement
Calvary UPC is a multi-building complex. The sanctuary and Family Connection Center offer a nice setting for weddings and receptions..
1.
While reservations are placed on the church calendar immediately upon request, they cannot be
confirmed until a signed contract and payment of security deposit is received.
2.
Full rental payment of $150 for the church and/or $150 for the FCC is required a minimum of 30 days
before the event. $50 of the deposit is non-refundable is paid for the sound technician and platform tear down and setup. The remaining $100 will be refunded within 30 days of event upon inspection of facility to determine that facility is left clean and with no damage. After the rental period, the Church will conduct a post-use inspection of the facilities and prepare a report that will note any breaches of contract and related deductions from the security deposit, or additional charges for damages, if any.

The facilities are not available for rent when a church ministry function is already scheduled.
4.
Activities that violate our church constitution and/or policies are not permitted at any time on the
premises. There is no smoking on the church premises (inside or outside), no dancing and alcoholic beverages of any kind or prohibited.
5.
If the renter desires for the church to be responsible for cleaning, there will be a $100 charge per building
used. All decorations must be removed by the renter prior to cleaning.
6.
Fees as provided by this document do not include any ministerial staff, wedding coordinator or
musicians/singers. Fees for those additional services must be made with the individuals involved.
7.
For reasons of personal safety and potential property damage, rice, bird seed confetti and similar
materials are not permitted inside or outside the church facilities. Wedding bubbles may be used, but must
remain outside of the building and all trash from them must be collected by someone in the wedding party/family.
User Responsibilities
All aspects of this agreement, including liability for damage/cleaning fees, are the responsibility of the contract signer. The contract signer is responsible for ensuring that all rules are followed.
1.
No food or beverages are allowed in the sanctuary. No open flames or burning of any substance, other
than candles, is permitted.
2.
A church event coordinator will be available to open and close the facility. All times for opening and closing
must be pre-arranged.
3.
The PA system and all other audio-visual equipment are to be operated by church staff only.
4.
When kitchen is used – stoves, sink, refrigerator, microwave and coffee pots must be left in the same clean
condition as found. Renters must supply their own consumables, table cloths, table settings and
decorations.
5.
Access will be granted to one classroom to allow for bridal party preparations. All other rooms are
considered off limits and entry will require extra charges.
6.
Damage to walls, floors, equipment and or furnishings of any room used during the time designated will be
billed to the contract signer.
7.
The telephone is for use of church business. No personal long-distance calls may be made from any church
phone.
8.
Smoking and the consumption of alcoholic beverages are prohibited throughout the church facility and
grounds. A Christian-like behavior and language are expected at all times. No illegal drugs are allowed on
church premises. Anyone suspected of being under the influence of alcohol or illegal drugs will be asked
to leave – there are no exceptions. No weapons shall be brought onto church property. No pets are allowed
with the exception of service animals.
9.
When attendance includes children and/or teenagers, the signer assures that adequate supervision will be
provided from the time the first person arrives until the last person leaves.
10.
No tape is to be applied to any painted or varnished surfaces. Decorations or displays and alterations to
existing room arrangements must have prior approval by the church staff.
11.
The contract signer is responsible for seeing that all members of the wedding party, guests, family and
outside contracted persons are aware of the church facility use policy. Any church property damaged or
destroyed become the full and complete responsibility of the contract signer.
Liability
1.
The Church staff will be responsible for interpretation and enforcement of all rules.
2.
The use of the church buildings and/or premises will be at the risk of the participant.
3.
Calvary UPC does not assume liability or responsibility for any participant injured during any event held at
our facility. “Play at your own risk” – all children must have adult supervision and be accompanied by an
adult at all times.
4.
The Church does not make any expressed or implied warranty of the premises, equipment, machinery,
fixtures or furniture.
By signature below, I agree that I have read and understand and will comply with all of the above listed rules and regulations. I further understand that my group, organization, event, party may incur damage assessment fees for any damage resulting from my usage of the facility. I agree to pay Calvary UPC for costs pertaining to clean up fees and/or damage fees as a result of my failure to comply with usage policy.

Usage Check List
1.
All equipment used has been replaced properly and security

2.
All electrical items have been turned off

Chairs have been replaced as found

Food, spills and trash have been removed

Sanctuary has been vacuumed

All trash and decorations have been removed from the church including no trash left in pews

7.
Basement/FCC floor has been swept and mopped

Counter tops have been cleared and wiped clean

Food items used have been removed from the kitchen (do not leave food in cabinets or in refrigerator)

10.
Bathrooms have been picked up, toilets flushed as needed and confirm that no toilets are still running

11.
Foyer area has been cleaned, decorations removed and floor vacuumed

12.
Foyer welcome table has been restored to original set up

13.
Classrooms used by bridal party have been cleaned, restored to original set up and no damage

14.
All trash, inside and outside, has been removed and liners replaced in trash cans.

15.
Platform is free from decorations and has been vacuumed (furniture and sound will be replaced by church 

staff)

16.
All personal belongings and/or rental equipment has been removed from the premises

17.
All doors have been secured, locked and all lights turned off

18.
Any church supplies used have been replaced or payment made for supplies
